What does RescueTime do when I have lots of apps and websites open at once?

RescueTime only tracks the application which is in focus or "on top"—or the tab you're working in at any given time. RescueTime only logs time for one application, website or document at a time, no matter how many tabs you have open in your browser or programs you have running.
